10/11/2008 Re: FDA Registration United States Food and Drug Administration Your facility has been successfully registered with the U.S. Food and Drug Administration ("FDA"), pursuant to the requirements of the U.S. Public Health Security and Bioterrorism Preparedness and Response Act of 2002. The FDA has issued your facility a Registration Number. This Registration Number must appear on every prior notice of every shipment of your products from your facility into the United States. Your broker or importer should be aware of this requirement now and should be given your Registration Number. If there is a change to any of the information you provided to us for your registration, now or at any time in the future, you are required by law to file an update with the FDA. Unless you notify us in writing to the contrary (by email on our website or by fax), all the information you provided to the FDA will be seen as accurate, true and correct by us and the FDA. As your authorized submitter to file the registration form on your behalf, we can also file any updates; simply let us know by email or fax and we can take care of that also. As your U.S. Agent, we will promptly and confidentially transmit any communications we receive from the FDA to you. Thank you for the opportunity to be of service.
